Clinical Co-ordinator Mental Health Specialist

We are seeking a highly skilled and compassionate Mental Health Practitioner to join our Community Mental Health Team. In this role, you will provide high-quality care to older adults with dementia and functional mental health conditions.
You will work collaboratively with our team to deliver personalized treatment plans, conduct comprehensive assessments, and provide support to patients and carers.
This is a rewarding and fulfilling position that requires a dedicated professional who is passionate about making a difference in the community.
Main Duties and Responsibilities
- Undertake assessments and coordinate care for service users
- Work closely with family members to maintain high standards of collaborative care
- Contribute to the day-to-day operational functions of the team, including completing new assessments and working as a team to deliver the team's goals
- Develop and maintain positive working relationships with service users, carers, General Practitioners, third sector partners, and other significant professionals
- Ensure clear channels of communication are established and maintained with clients, carers, GPs, Social Services, and other partners

Person Specification
- Education to diploma/degree level and registered with the appropriate professional body
- Professional knowledge acquired through a relevant degree plus relevant experience for working with complex cases
Desirable Criteria
- Evidence of continued professional development
- NVQ A 1 qualification
- Relevant mentorship qualification as required by governing body
- Specialist practitioner qualification (SPQ)
