As a member of our heart failure team, you will play a crucial role in facilitating the early identification of inpatients admitted with suspected or confirmed heart failure. This involves working closely with community patient clients to provide support and guidance on outpatient clinics and home visits.

Key Responsibilities

This role includes:

  • Supporting the community patient clients in running outpatient clinics and conducting relevant home visits.
  • Providing clinical leadership, advice, and support to staff on all aspects of heart failure care.
  • Developing a care pathway that improves discharge planning and discharge information.

**Annual Salary:** £47,600 (based on a 37-hour week)
