Physical Rehabilitation Assistant

The Rehabilitation Assistant will work as part of a multidisciplinary team to provide high-quality care and support to patients in a community-based setting. The successful candidate will be responsible for developing and implementing personalized care plans, promoting independence and quality of life for patients.
The ideal candidate will possess excellent communication and interpersonal skills, knowledge of patient conditions, and experience in physical/functional rehabilitation of elderly/adults. They will also possess IT literacy and the ability to exert physical effort as required by the role.
Key Responsibilities:
- Conduct thorough assessments of patient needs, identifying areas for improvement and developing targeted care plans.
- Deliver high-quality care and support, including exercises, therapies, and mobility training.
- Maintain accurate records of patient progress, communicating effectively with multidisciplinary teams.
Requirements:
- Candidates must possess excellent communication and interpersonal skills.
- A genuine interest and commitment to providing high-quality care to predominantly older people.
- Demonstrated ability at exercising tact and diplomacy.
- Demonstrate a caring, compassionate, and friendly attitude.
Salary: £25,000 - £35,000 per annum (dependent on experience)
