We are seeking a highly skilled Social Worker - Adults with Disabilities Specialist to join our team at Harrow Council. The ideal candidate will have a strong background in care act assessments, support planning, and reviews. Key responsibilities include:

  1. Care Act Assessments
  2. Support Planning and Reviews
  3. CHC Assessments
  4. Asylum/NRPF, Transition, Safeguarding
  5. Undertaking comprehensive social care assessments and reports, with detailed support plans, as appropriate.
  6. Knowledge and understanding of Direct Payments using the strength-based approach.
  7. Sound knowledge around the continuing health care process and procedure and the legislation that underpins this, including the Decision Support Tools.

As a Qualified Social Care Professional - Adults with Disabilities, you will be responsible for providing high-quality care and support to adults with disabilities. You will work closely with the community, healthcare professionals, and other stakeholders to ensure the best possible outcomes for our service users.

The successful candidate will have:

  • SWE Registration
  • 1 year post-qualifying experience within the UK Social Care industry
  • Enhanced DBS/CRB (we can obtain for you)
  • Eligibility to work in the UK
