Job Overview

We are seeking a skilled Senior Support Worker to join our team at NELFT North East London Foundation Trust. This is an exciting opportunity for a motivated individual to work in a dynamic environment, providing support to service users in the community.

In this role, you will be responsible for working as part of a multi-disciplinary team to provide evidence-based care that adheres to agreed policies and procedures. Your primary goal will be to contribute to the effectiveness of services by working with patients and families to assess risks and needs, acting as patient advocates, and implementing care plans or directives overseen by registered nurses.

To succeed in this position, you will need to have relevant experience in a community setting, including lone working and experience of working with a multidisciplinary team. You should also possess knowledge and understanding of recovery and social inclusion, as well as the ability to listen effectively and communicate effectively at all levels.

The ideal candidate will be aware of local services and resources, have knowledge of benefits and employment systems, and be able to travel within the geographical work area and across directorates. Additionally, they should be flexible, supportive, and able to take on new roles within the organization as required.

Key Responsibilities

• Work as part of a multi-disciplinary team to provide evidence-based care that adheres to agreed policies and procedures
• Assess risks and needs, act as patient advocates, and implement care plans or directives overseen by registered nurses
• Develop and maintain effective relationships with patients, families, and other healthcare professionals
• Participate in training and professional development opportunities to enhance skills and knowledge
• Contribute to the implementation of care plans by helping to assess patients, develop, implement, and evaluate a care plan

Qualifications and Experience

• NVQ level 3 in care or equivalent
• Relevant experience in a community setting, including lone working and experience of working with a multidisciplinary team
• Knowledge and understanding of recovery and social inclusion
• Ability to listen effectively and communicate effectively at all levels
• Awareness of local services and resources
• Knowledge of benefits and employment systems
