Clinical Triage Practitioner

We are seeking a highly skilled Clinical Triage Practitioner to join our team at Queen Elizabeth's Foundation for Disabled People. As a key member of our rehabilitation services, you will play a crucial role in assessing and triaging patients with complex neurological conditions.
Key Responsibilities- Assessment and Triage: Conduct thorough assessments of patients with neurological conditions, using your expertise to identify their rehabilitation needs and develop personalized plans.
- Communication and Collaboration: Work closely with multidisciplinary teams, including healthcare professionals, therapists, and carers, to ensure seamless communication and coordination of care.
- Risk Management: Identify and mitigate risks associated with patient care, ensuring a safe and supportive environment for all.
- Professional Development: Stay up-to-date with the latest clinical practices and research, applying your knowledge to improve patient outcomes and services.
- Leadership and Guidance: Provide guidance and support to junior staff members, promoting a culture of excellence and continuous learning.
- Qualifications: Registered nurse or postgraduate physiotherapist/occupational therapist with current NMC or HCPC registration.
- Experience: 5+ years of post-qualification clinical experience, with a minimum of 3 years in community or rehabilitation settings.
- Skills: Excellent communication and interpersonal skills, with the ability to work autonomously and as part of a team.
