We are seeking a highly skilled Physiotherapist to join our team as a Rehabilitation Specialist in Primary Care. As a key member of our multidisciplinary team, you will be responsible for managing a complex caseload of patients, providing expert clinical leadership, and contributing to the development of our services.
Key Responsibilities- Assess, diagnose, and manage patients with complex musculoskeletal conditions, taking responsibility for their care and rehabilitation.
- Develop and implement individualized treatment plans, incorporating evidence-based practice and patient-centered care.
- Work collaboratively with other healthcare professionals to ensure seamless care pathways and optimal patient outcomes.
- Contribute to the development and implementation of clinical guidelines and protocols, ensuring best practice in musculoskeletal care.
- Provide education and training to patients, carers, and healthcare professionals, promoting self-management and empowerment.
- Participate in quality improvement initiatives, evaluating and improving the effectiveness of our services.
- Postgraduate qualification in Physiotherapy (MSc or equivalent).
- Registration with the Health and Care Professions Council (HCPC).
- Experience in musculoskeletal physiotherapy, with a strong understanding of complex caseload management.
- Excellent communication and interpersonal skills, with the ability to work effectively in a multidisciplinary team.
- Ability to work autonomously, with a high degree of self-motivation and initiative.
- Commitment to ongoing professional development, with a desire to stay up-to-date with best practice in musculoskeletal care.
