If so then we are looking to recruit an enthusiastic and highly motivated individual to join our engaged and committed Community Rehabilitation and Falls Service at the Midlands Partnership University Foundation Trust.

Our rehabilitation teams are working in many innovative and exciting ways to positively influence their provision of care and push the boundaries of effective service delivery.

Our multi-professional and integrated teams have demonstrated this through their recent success for the Catalyst Funding from the regional Stroke Quality Improvement for Rehabilitation team to improve the life of Stroke Survivors in the area.

Across the Community Rehabilitation teams, the clinical team consists of Advanced Physiotherapists, Physiotherapists, Occupational therapists, Assistant Practitioners and Specialist Nurses. The Community Rehab teams within this portfolio are based in South East Staffordshire and work very closely with other community rehab teams across the region to ensure governance across the patch and reduce any unwarranted variation. These close relationships provide a vast support network and wealth of knowledge including cross team learning, peer support, evidence base training days, critically appraised topic groups and links to researchers from Keele University.
- As part of the job role you will be required support colleagues and contribute to departmental and service needs.
- The post holder will be required to have excellent interpersonal and communication skills, show initiative and team spirit (strong sense of team / collaborative working) as well as be committed to patient care.Continuing Professional Development is encouraged and identified through annual appraisal. Professional supervision is offered through groups or individual supervision as appropriate.
- Operationally manage a multi-disciplinary team, in accordance with Trust values, in the delivery of effective, efficient and high-quality services for service users.
- Work in conjunction with the General Manager, Operational Manager, Head of Operations, Clinical Lead and Professional Leads to develop and deliver safe and effective services, in accordance with key national targets, clinical standards, service priorities and performance indicators set by local commissioners and Trust policies.
- You will support and oversee operational delivery of services at team level; they will be required to work autonomously in order to address issues impacting on service delivery with overall support and strategic direction from the General Manager / Operational Manager.
- You should have access to a car for work purposes.

KEY RESPONSIBILITIES

Leadership / Service Development
- Manage the team in accordance with Trust values and Care Group objectives.
- Work with colleagues to develop and communicate the vision of patient centred care.
- Participate in the formulation of operational plans; which take into account local, regional and national drivers and support their safe and effective implementation.
- Develop local objectives and plans within the team in conjunction with the General Manager to deliver services in line with the integrated care agenda and Care Group objectives.
- Analyse team data and information relating to performance to inform future service priorities.
- Ensure active service user involvement is embedded within the team to support service development and innovations.
