We have an exciting and rare 12-month secondment or fixed term opportunity for an Advanced Stroke Specialist with the Southeast Staffordshire Community Rehabilitation team.

We require your skills in research, analysis and project evaluation to support formulating a business case for continued funding. You will support the development of audit and research skills, within the team to evidence the efficacy of the pilot pathway.

This post is open to Registered Allied Health Professionals and NMC Registered Nurses with advanced knowledge and skills in caring for patients with a diagnosis of stroke.

Advanced Stroke Specialist will work closely with the Stroke Project Coordinator and the project team to contribute to the development of a stroke rehabilitation service having a particular focus on research and innovation, whilst also supporting service design and improvements.

The Southeast Staffordshire Improving Lives After Stroke Project has been funded by SquIRe for 12 months. During the first 6 months you will be involved in scoping existing pathways and establishing how the project can evidence primary and secondary outcomes. The second 6 months will focus on delivery of a pilot pathway and evaluation.

Due to the nature of the work you will be required to have travel independently across the Staffordshire geographical patch. Ideally, you will hold a full driving licence and have access to a car.

The role will take an integrated working approach liaising with our partners in secondary, primary and community care settings as well as the voluntary sector to ensure the patient pathway has a strong focus on patient centred care.

**As part of the role you will**:
Identify and provide support for the Stroke Project Co-ordinator in networking and working with relevant services including the Acute Trust, ICB, Social Services, the independent sector, and patients / carers.
Support the Stroke Project Team with the development of clinical, educational, leadership and research skills.
Educate clinicians in audit skills, service development, research methodology and statistics so that critical appraisal and the use of research becomes embedded within clinical practice.
Ensure that appropriate research findings are translated into practice, disseminated to staff and patients, and implemented into best practice.
Build new and maintain existing effective working relationships and networks within strategic and operational models of working.
Write and submit nominations for relevant projects, audits, research, teams, and individual staff members to MPFT, national and international awards.
Liaise effectively with key front-line practitioners. Support developing stroke aftercare and review in a community setting for stroke survivors, family members and/or carer.
Ensure secondary prevention and support for stroke patients in the community following their initial episode of care.
