About the Role:

  • We are seeking an experienced Care Coordinator to provide high-quality care coordination services to our clients.
  • You will work closely with our care teams to ensure seamless delivery of care services.

Key Responsibilities:

  • Develop and implement individualized care plans for clients.
  • Co-ordinate and manage care staff to ensure effective delivery of care services.

Requirements:

  • Proven experience as a Care Coordinator or in a related field.
  • Degree in Social Work or a related field (desirable).
  • Excellent communication and interpersonal skills.
