Care Coordination Role

At The Recruitment Crowd (Yorkshire) Limited, we are seeking a dedicated and organized individual to fill the position of Care Coordinator.

About the Job:

We are looking for an experienced professional with excellent organizational skills to manage our home care services in Southport. As a Care Scheduler, you will be responsible for coordinating home care visits with our team of caregivers.

Main Responsibilities:

  • Coordinating home care visits and scheduling appointments.
  • Maintaining effective communication between clients, caregivers, and healthcare professionals.
  • Managing client relationships and addressing any concerns or issues that may arise.
  • Accurately recording client interactions and care plans.
  • Collaborating with the care team to deliver high-quality services.

Requirements:

  • A relevant qualification in care coordination or a related field.
  • Exceptional organizational and time management skills.
  • Strong communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Basic computer literacy and proficiency in Microsoft Office.
