Job Title

Care Coordinator Specialist

We are seeking an experienced Care Coordinator to join our team in Southport. The ideal candidate will have previous experience in domiciliary care, strong IT skills, and a passion for delivering person-centred care.

About the Role
  • Producing and managing staff rotas to ensure all calls are covered
  • Liaising with hospitals, GPs, social services, and families to provide exceptional care
  • Conducting assessments and reviews to maintain top care standards
  • Managing on-call duties and weekend work on a rota basis

This is a challenging and rewarding role that requires a naturally proactive, compassionate individual with a strong commitment to excellence.

Requirements
  • A full driver's licence and access to a vehicle
  • Previous experience in rota management and scheduling, and a proven track record in coordinating within a domiciliary care setting

The estimated salary for this role is £25,000-£26,000 per annum, depending on experience.
