Job Description: Care Coordinator Specialist

We're currently seeking a skilled Care Scheduler in Southport to join our esteemed Home Care provider. This is an excellent opportunity to take your career in care coordination to the next level.

Key Responsibilities:

  • Producing and managing staff rotas to guarantee all calls are covered.
  • Liaising with hospitals, GPs, social services, and families to ensure the best possible care for service users.
  • Conducting assessments and reviews to maintain top-notch care standards.
  • Managing on-call duties and weekend work on a rota basis.

Requirements:

  • A full driver's license and access to a vehicle.
  • Strong IT skills, exceptional organisational abilities, and a passion for delivering person-centred care.
  • Previous experience in domiciliary care and rota management is essential.
