Role Overview

Choices Homecare is seeking a skilled Care Coordinator to join our team at the Southport office. As a Care Coordinator, you will play a vital role in delivering high-quality, user-focused domiciliary care services.

Key Responsibilities

  • Coordinate and deliver domiciliary care services to meet the needs of service users.
  • Manage a team of Care Workers, ensuring excellent service delivery through performance management and staff supervision.
  • Liaise with hospitals, GPs, social services, and families to ensure the best care for service users.
  • Conduct assessments and reviews to maintain top care standards.
  • Manage on-call duties and weekend work on a rota basis.

Requirements

  • Previous experience in domiciliary care is essential.
  • Strong IT skills, exceptional organisational abilities, and a passion for delivering person-centred care.
  • A full driver's licence and access to a vehicle.
