About the Role

We are seeking a skilled and compassionate Community Response Support Worker to join our team at Derbyshire Community Health Services NHS Foundation Trust. As a key member of our Community Response Team, you will play a vital role in supporting individuals in their own homes, providing high-quality care and promoting independence.

Key Responsibilities
  • Deliver person-centered care to patients in their own homes, working collaboratively with clinicians and other healthcare professionals.
  • Assist patients with daily living tasks, promoting independence and dignity.
  • Contribute to the development of care plans, ensuring patient needs are met and goals are achieved.
  • Work effectively in a fast-paced environment, prioritizing tasks and managing time efficiently.
  • Communicate effectively with patients, families, and healthcare teams, providing clear and concise information.
Requirements
  • NVQ Level III in care or equivalent qualification.
  • Experience working in a healthcare setting, preferably in a community-based role.
  • Excellent communication and interpersonal skills, with the ability to work effectively with diverse individuals.
  • Ability to maintain accurate records and contribute to the development of care plans.
  • Valid UK driving license and access to a vehicle, with a willingness to travel across the county.
