About the Role

We are seeking a highly skilled and compassionate Mental Health Practitioner to join our team at Southern Health NHS Foundation Trust. As a key member of our multidisciplinary team, you will play a vital role in providing high-quality care to individuals experiencing mental health crises and those with mental health needs during their inpatient stay in the acute hospital.

The Mental Health and Learning Disability Liaison Team provides assessments, advice, and reasonable adjustments for people with mental health needs and learning disabilities accessing treatment at St Mary's Hospital. Our team is dedicated to improving the parity of care for people of all ages with mental health and learning disabilities in the acute hospital and working with their family and carers to ensure they receive the best possible care and treatment.

Key Responsibilities
  • Assess and manage risk, intervene, and provide crisis management for individuals experiencing mental health crises.
  • Develop and implement effective care plans that address the social and health care needs of individuals.
  • Work collaboratively with multidisciplinary teams to provide comprehensive care and support.
  • Provide education and training to staff and patients on mental health and learning disability issues.
  • Participate in quality improvement initiatives to enhance patient care and outcomes.
Person Specification

We are looking for a qualified and experienced Mental Health Practitioner who possesses the following skills and qualities:

  • Registered Nurse or Allied Health Professional with a background in mental health.
  • Experience in assessment, risk management, and crisis intervention.
  • Knowledge of mental health models of practice and interventions.
  • Ability to work autonomously and as part of a team.
  • Excellent communication and interpersonal skills.
