About the Role

We are seeking a highly skilled and compassionate Mental Health Practitioner to join our team at Southern Health NHS Foundation Trust. As a key member of our multidisciplinary team, you will play a vital role in providing high-quality care to individuals experiencing mental health crises and those with learning disabilities.

As a Mental Health Practitioner, you will be responsible for assessing and managing risk, developing and implementing care plans, and providing support and guidance to patients and their families. You will work closely with other healthcare professionals to ensure seamless care and effective communication.

We are looking for an individual who is passionate about delivering exceptional patient care, has excellent communication and interpersonal skills, and is able to work effectively in a fast-paced environment.

Key Responsibilities
  • Assess and manage risk, developing and implementing care plans to meet individual needs
  • Provide support and guidance to patients and their families, promoting independence and well-being
  • Work collaboratively with other healthcare professionals to ensure seamless care and effective communication
  • Participate in multidisciplinary team meetings to discuss patient care and develop care plans
  • Contribute to the development and implementation of policies and procedures to improve patient care and outcomes
Person Specification

We are looking for an individual who:

  • Has a degree in a relevant field (e.g., nursing, occupational therapy, social work)
  • Has experience working in mental health settings, preferably in a crisis or acute care environment
  • Has excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare professionals
  • Is able to work autonomously and as part of a team, with a flexible approach to working hours and shifts
  • Has a clean driving license and access to a vehicle (for travel between sites)
