Mental Health Practitioner

We are seeking a skilled Mental Health Practitioner to join our Crisis Resolution Home Treatment Team on the Isle of Wight. As a key member of our multi-professional team, you will work to provide intensive treatment and support to individuals in crisis, promoting recovery and well-being in the community.
Main Responsibilities- Conduct comprehensive assessments of individuals' mental health needs, identifying symptoms and developing effective treatment plans.
- Work collaboratively with other healthcare professionals to provide a coordinated response to individuals in crisis, ensuring seamless transitions between services.
- Provide expert advice on medication and health issues, administering or supervising medication as required.
- Develop and implement individualized care plans, focusing on empowerment and recovery.
- Contribute to the development of the team's knowledge and skills, sharing best practices and innovations in mental health care.

Person SpecificationWe are looking for a qualified Mental Health Practitioner with experience of working with individuals who have complex mental health needs. You will have a strong understanding of mental health theories and models, as well as excellent communication and interpersonal skills. A background in crisis resolution and home treatment is highly desirable.
