We are seeking a highly skilled and motivated Primary Care Mental Health Practitioner to join our team at Essex Partnership University NHS Foundation Trust. As a Primary Care Mental Health Practitioner, you will play a vital role in providing high-quality mental health services to patients in the community.
About the Role:- Work in conjunction with GPs, primary care teams, and consultant psychiatrists to ensure seamless patient care.
- Provide physical health checks for patients on the SMI register and support patients discharged from secondary care.
- Collaborate with the Enhanced Primary Care team to deliver integrated, holistic care.
- Develop and maintain strong relationships with patients, carers, and community partners.
