About the Role
Southern Health NHS Foundation Trust is seeking a skilled and compassionate Healthcare Support Worker to join our Community Mental Health Team at Avalon House in Winchester.
As a key member of our team, you will play a vital role in delivering high-quality care to individuals with complex mental health needs. Your primary responsibilities will include providing physical and psychological care, supporting the planning and delivery of interventions, and working collaboratively with individuals, families, and other healthcare professionals.
Main Duties
- Provide physical and psychological care to service users in accordance with the specified plan of clinical care and in line with the recovery model.
- Carry out duties as designated by the team leader.
- Support the planning and delivery of interventions in line with an agreed care plan.
- Deliver higher level intensity community interventions as part of the integrated Shared Care model.
- Work in a manner which is helpful to the service user and the referrer, based on principles of recovery, personalisation, and choice.
- Work with individuals in the least restrictive manner possible.
- Have previous experience in working with people with mental health needs and be able to evidence effective team working.
- Complete phlebotomy training to take bloods for our clozapine clinic.
- Ability to drive and use of a car for work purposes is essential in this role.
- Provide support to the person responsible for the clinical care of the service user.
- Accurately record service user information.
- Provide support to relatives/carers.
