About the Role

We are seeking an experienced Care Coordinator Specialist to join our team at Primary Care 24. As a key member of our support team, you will be responsible for providing exceptional care navigation services to patients.

Job Description
  • Key Responsibilities:
  • Provide high-quality care navigation call handling duties in line with PC24 processes.
  • Communicate effectively with patients and the primary care team to ensure seamless care coordination.
  • Action follow-up calls and provide operational support to the onsite practice team and clinical team.
  • Ensure patient safety by following established processes and procedures.
  • Record essential information accurately and concisely.
  • Utilize effective questioning and active listening skills to achieve optimal patient outcomes.
Requirements
  • A minimum of GCSE Level C in literacy and numeracy or equivalent work-based experience.
  • Exceptional communication skills with diverse individuals.
  • Proficiency in Microsoft Office packages and excellent keyboard skills.
  • Patient-focused skills with the ability to demonstrate patience, empathy, and compassion via telephone.
Estimated Salary

The estimated salary for this role is £25,000 - £30,000 per annum, depending on experience.
