About the Position

We are looking for a highly motivated Practitioner to join our team, working with children and young adults with complex needs. As a Practitioner, you will be responsible for providing individualized support and care.

Key responsibilities include:

  • Developing and implementing personalized care plans.
  • Collaborating with multidisciplinary teams to ensure comprehensive support.
  • Providing emotional support and promoting independence.
  • Maintaining accurate records of client progress and needs.

Requirements

  1. A strong background in social care or a related field.
  2. Excellent communication and interpersonal skills.
  3. Ability to work effectively in a fast-paced environment.
  4. Basic computer skills and familiarity with electronic record systems.
  5. A Diploma Level 3 in Young People's Workforce (or equivalent) is desired.
