**Job Description**

As a Social Worker in the Adults Localities Team, you will be responsible for conducting assessments, developing care plans, and collaborating with other professionals to provide high-quality support to older adults and individuals with physical disabilities. Your goal will be to empower individuals to maintain their independence and improve their overall quality of life.

  1. You will conduct thorough assessments to understand individuals' needs, risks, and strengths.
  2. You will work closely with service users, their families, and other professionals to create personalized care plans.
  3. You will collaborate with multi-disciplinary teams to deliver a holistic approach to care.
  4. You will ensure adherence to relevant legislation, policies, and procedures.

Qualifications and Requirements

To be successful in this role, you will need:

  • A degree in Social Work or equivalent qualification.
  • Registration with Social Work England.
  • Experience working with older people and/or individuals with physical disabilities.
  • Strong assessment and care planning skills.
  • Knowledge of relevant legislation and best practices in social work.
