About the Role
We are seeking a skilled and compassionate Registered Nurse to join our team as a Community Staff Nurse. This is an exciting opportunity to work in a dynamic and supportive environment, providing high-quality care to patients in their own homes.
Key Responsibilities
- Provide holistic assessments of patients' care needs and develop individualized care plans.
- Deliver high-quality care to patients, including medication administration, wound management, and palliative care.
- Work collaboratively with other healthcare professionals to ensure seamless care delivery.
- Participate in multidisciplinary team meetings to discuss patient care and share best practices.
Requirements
- Registered Nurse qualification and current registration with the NMC.
- Experience working in a community setting, preferably in a district nursing role.
- Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
- Ability to work autonomously and as part of a team, with a strong focus on patient-centered care.
