Community Health Nurse Leader

Job Overview
We are seeking an experienced and skilled Community Health Nurse to join our team at Kent Community Health NHS Foundation Trust. As a Community Health Nurse, you will be responsible for delivering high-quality patient care in a community setting.
Main Duties
Key responsibilities include:
- Providing expert clinical care to patients in their homes or community settings
- Working closely with other healthcare professionals to ensure seamless care delivery
- Developing and implementing care plans to meet individual patient needs
- Conducting assessments and evaluations to inform care decisions
- Collaborating with community teams to promote patient self-management and independence
Requirements
To be successful in this role, you will need:
- Registered health care professional (NMC registration)
- Health-related degree and evidence of relevant postgraduate development at Level 6
- Specialist qualifications relevant to area of practice (e.g. specialist practitioner district nursing, non-medical prescribing)
