We are seeking a dedicated and enthusiastic Care Team Coordinator to join our Primary Care team at HMP Askham Grange, a female open prison and YOI in North Yorkshire.

This is an exciting opportunity to develop your skills and experience in a secure environment, working with a diverse range of patients and staff.

About the Role

As a Care Team Coordinator, you will be responsible for coordinating care packages, assisting with health promotion and wellbeing initiatives, and supporting the nursing and pharmacy teams in primary care.

You will have the opportunity to work closely with the multidisciplinary team, developing care plans and ensuring seamless care delivery.

Key Responsibilities
  • Coordinate care packages, tailoring support to meet individual patient needs
  • Assist with health promotion and wellbeing initiatives, promoting healthy lifestyles and disease prevention
  • Support the nursing and pharmacy teams in primary care, ensuring seamless care delivery
  • Develop and implement care plans, in collaboration with the multidisciplinary team

We offer a competitive salary of £15.00 per hour, plus benefits including bespoke induction, competency framework, regional and national career development opportunities, and support from the wider team.

Requirements
  • NVQ Level 3 qualification in Health & Social Care, or equivalent
  • Excellent communication skills, with a non-judgemental and compassionate approach
  • Ability to work effectively in a fast-paced environment, prioritising tasks and managing time efficiently
  • Driving license and own vehicle desirable, due to site location
