Care Coordination Role

We are looking for an experienced Integrated Specialist Care Coordinator to work in the North team, covering the Pocklington/Market Weighton area within the West Wolds Team.

The successful applicant will lead the delivery of the Healthy Child Programme for children aged 0-9 months to 11 years and be based in Pocklington.

Main Duties:
  • Coordinate the Integrated Specialist Public Health Nursing Team (0-9 months - 11 years) and ensure effective integration into a single service.
  • Deliver care packages to children, young people, and their families, including the Healthy Child Programme.
  • Manage a caseload or client group, compile reports, and participate in multi-agency safeguarding procedures.
  • Provide coordination and supervision of team members and undertake delegated management tasks.
  • Offer specialist advice in areas such as perinatal Mental Health and Infant Mental Health.

Required Skills and Qualifications:Qualifications:
  • NMC registration.
  • Specialist Community Public Health Nursing qualification.
  • Teaching qualification or equivalent presentation skills/training experience.
  • Non-medical prescribing qualification.
Experience:
  • Relevant continuing professional development.
  • Case load management experience within Children's Services.
Knowledge:
  • Understanding of policy, legislation, and guidance relevant to the clinical area.
  • Immunization skills.
Estimated Salary: £52,000 per annum
