About the Role

We are seeking a highly skilled and dedicated Designated Keyworker to join our team at Lancashire Care Foundation Trust. As a Designated Keyworker, you will play a vital role in supporting children, young people, and adults with Autism Spectrum Disorder (ASD) and learning disabilities who are in crisis.

The Designated Keyworker Team provides a multi-agency approach to support individuals and their families, working closely with statutory and non-statutory agencies to ensure a coordinated and person-centered approach to planning and support.

Key Responsibilities
  • Work with children, young people, and adults with ASD and learning disabilities who are in crisis, providing emotional support and guidance to help them manage their needs.
  • Develop and implement individualized support plans, working closely with families, carers, and other professionals to ensure a comprehensive and coordinated approach to support.
  • Provide training and education to families, carers, and other professionals on ASD and learning disabilities, promoting understanding and awareness of the needs of individuals with these conditions.
  • Collaborate with other agencies and professionals to ensure a seamless and effective service, working to achieve the best possible outcomes for individuals and their families.
Requirements
  • Education to degree level in a relevant subject or equivalent level of qualification or significant equivalent previous proven lived experience.
  • Experience of working with children who have complex needs and their families, including experience of multi-agency working.
  • Knowledge of systems and processes, including understanding of statutory requirements for children and young people across various agencies.
