Designated Keyworker

Lancashire Care Foundation Trust is seeking a highly skilled and dedicated Designated Keyworker to join our team. As a Designated Keyworker, you will play a vital role in supporting children, young people, and adults with Autism Spectrum Disorder (ASD) and learning disabilities who are in crisis.
The Designated Keyworker Team provides a multi-agency approach to support individuals and their families, working closely with statutory and non-statutory agencies. You will be responsible for managing a caseload, providing person-centred support, and coordinating with other professionals to ensure the best possible outcomes for our service users.
Key Responsibilities- Provide person-centred support to individuals with ASD and learning disabilities
- Manage a caseload of complex needs and families
- Work collaboratively with other professionals to ensure seamless support
- Develop and implement individualised support plans
- Monitor and evaluate the effectiveness of support plans
To be successful in this role, you will need:
- Excellent communication and interpersonal skills
- Ability to work autonomously and as part of a team
- Experience of working with children and adults with complex needs
- Knowledge of systems and processes
- Understanding of statutory requirements for children and young people
