Physiotherapist - Community Rehabilitation Specialist

About the Role
We are seeking a highly skilled and motivated Physiotherapist to join our Joint Community Rehabilitation Service (JCR) team in Lewes and Eastbourne. As a Band 6 Physiotherapist, you will play a key role in assessing and treating patients in their own homes, preventing unnecessary hospital admissions and promoting independence and quality of life.
Main Responsibilities
- Assess and treat patients in their own homes, using a client-centred approach to rehabilitation.
- Work collaboratively with a multidisciplinary team, including Occupational Therapists and Support Workers, to provide comprehensive care.
- Develop and implement individualised rehabilitation plans, taking into account the patient's medical and surgical conditions.
- Provide training and supervision to junior staff members, promoting a culture of learning and development.

Person Specification
- Essential qualifications: Degree or diploma in Physiotherapy, Health and Care Professions Council Registration, post-graduate qualification or proven equivalent experience and knowledge in relevant field.
- Essential experience: 18 months - 2 years rotational Band 5 experience, experience of training/supervision, experience of multi-professional working, experience of using client-centred goals.
