About the Role

We are seeking a highly skilled and experienced Physiotherapist to join our Integrated Health and Social Care Community Rehabilitation Service in Lewes. As a Band 6 Physiotherapist, you will play a key role in assessing and treating patients in their own homes, preventing unnecessary hospital admissions and promoting independence and quality of life.

Key Responsibilities

  • Assess and treat patients in their own homes, using a client-centred approach
  • Work collaboratively with a multi-professional team, including Occupational Therapists and Support Workers
  • Develop and implement individualised rehabilitation plans, using evidence-based practice
  • Provide training and supervision to junior staff members
  • Contribute to the development of the service, through participation in quality improvement initiatives and research projects

Person Specification

  • Degree or diploma in Physiotherapy
  • Health and Care Professions Council Registration
  • Post-graduate qualification or proven equivalent experience and knowledge in relevant field
  • 18 months - 2 years rotational Band 5 experience
  • Experience of training/supervision
  • Experience of multi-professional working
  • Experience of using client-centred goals
