Physiotherapist - Community Rehabilitation Specialist

About the Role
We are seeking a highly skilled and experienced Physiotherapist to join our Integrated Health and Social Care Community Rehabilitation Service in Lewes. As a Band 6 Physiotherapist, you will play a key role in assessing and treating patients in their own homes, preventing unnecessary hospital admissions and promoting independence and quality of life.
Key Responsibilities
- Assess and treat patients in their own homes, using a client-centred approach
- Work collaboratively with a multi-professional team, including Occupational Therapists and Support Workers
- Develop and implement individualised rehabilitation plans, using evidence-based practice
- Provide training and supervision to junior staff members
- Contribute to the development of the service, through participation in quality improvement initiatives and research projects

Person Specification
- Degree or diploma in Physiotherapy
- Health and Care Professions Council Registration
- Post-graduate qualification or proven equivalent experience and knowledge in relevant field
- 18 months - 2 years rotational Band 5 experience
- Experience of training/supervision
- Experience of multi-professional working
- Experience of using client-centred goals
