About the Role:Key Responsibilities:
- Case Management: You will be responsible for managing a caseload of 15 young people with moderate to acute mental health issues, providing them with personalized support and guidance to achieve their goals.
- Pathway Planning: You will work with young people to develop robust and detailed Pathway Plans that help them achieve their goals, which may include group activities and individual work.
- Collaboration and Liaison: You will work closely with the Therapeutic Service, Transitions Social Worker, and Adult's Social Care Social Worker/Care Coordinators to ensure a comprehensive and coordinated approach to supporting young people.
- Assessment and Reporting: You will conduct assessments, including risk assessments, and collect and analyze relevant information to inform your work with young people.
- Record Keeping: You will maintain accurate and up-to-date children's case records in compliance with Council procedures and quality standards.
Requirements:
- Qualified Social Worker with experience of working with the Care Act 2014 and within Adult Mental Health Services.
- Effective oral and written communication skills, including formal report writing.
- Ability to handle challenges and own workload with the commitment to achieve positive outcomes.
- Proven experience in carrying out assessments, including risk assessments, collecting, and analyzing all relevant information and following guidance.
