About This Opportunity

We are seeking an experienced Child and Family Practitioner to join our team at Guidant Global. The successful candidate will be responsible for delivering high-quality care and support to children, young people, and their families.

The ideal candidate will have a solid understanding of social care and health within the community, as well as excellent communication and interpersonal skills. You will work collaboratively with colleagues to ensure individuals receive comprehensive care and support.

Key Responsibilities
  1. Assessment and Intervention
  • You will conduct thorough assessments to understand the needs of individuals and their families.
  • You will develop and implement effective interventions to meet identified needs.
Safeguarding and Risk Management
  • You will identify and address potential risks and safeguarding concerns in a timely manner.
  • You will collaborate with colleagues to ensure effective risk management practices.
Partnership Working
  • You will build and maintain strong relationships with partner agencies to ensure seamless care and support.
Independence and Autonomy
  • You will empower individuals to achieve independence and autonomy through tailored support.
  • You will use a person-centred approach to develop creative solutions.
Requirements
  • Essential qualifications include a clear understanding of social care and/or health within the community.
  • Able to demonstrate excellent communication and interpersonal skills.
  • Proficient in written and IT skills.
