Dementia Support Practitioner

We are seeking a dedicated and compassionate Care Assistant to join our team at St Monica Trust.
As a Care Assistant, you will be responsible for providing high-quality care and support to our residents, ensuring they receive the best possible experience.
Your main responsibilities will include:
- Assisting residents with their daily needs, including personal care and medication management.
- Providing emotional support and building strong relationships with residents and their families.
- Working collaboratively with the healthcare team to ensure seamless care delivery.
Salary: £12.43 per hour, plus enhancements where applicable.
Key Responsibilities:
- Delivery of day-to-day holistic care for residents and meeting their unique needs.
- Working with volunteers, wellbeing coordinators, and therapy teams to provide meaningful activities.
- Building relationships with stakeholders, including key worker relationships with named residents.
