About the Role

We are seeking an experienced Clinical Triage Practitioner to join our team in the Learning Disability service. The successful candidate will be responsible for triaging incoming referrals to determine eligibility for our services and contributing to the effective provision of care to individuals with complex needs.

Key Responsibilities
  • Triage incoming referrals to assess eligibility for our services
  • Liaise with stakeholders, including care providers, GPs, and families, to assess patient needs and determine eligibility
  • Communicate complex information to patients, families, and carers in a clear and concise manner
  • Work collaboratively with multidisciplinary teams to ensure consistent and coordinated care
  • Participate in safeguarding and contribute to the development of Specialist Learning Disability Services
Requirements
  • Diploma or Degree in Community Practice, Behavioural Approaches, or Family and Systemic Therapy
  • IT skills and evidence of ongoing training and development
  • Ability to work in a fast-paced environment and prioritize tasks effectively
  • Excellent communication and interpersonal skills
