Care Coordinator for Vulnerable Infants and Families

We are seeking a highly skilled and dedicated social worker to join our innovative team in Camden, working within the context of Infant Mental Health. The successful candidate will be part of a multi-disciplinary team, conducting collaborative assessments and interventions with vulnerable infants and their families.
Job OverviewThis exciting opportunity offers a challenging yet rewarding role, allowing you to contribute to service development and make a positive impact on the lives of infants, children, and their families. You will have a manageable caseload, excellent training and development opportunities, and the chance to drive improved outcomes through your practice.
Key Responsibilities- To work collaboratively with a multi-disciplinary team to conduct assessments and develop plans that support the needs of vulnerable infants and their families
- To identify and manage risks, ensuring the safety and well-being of all individuals involved
- To maintain accurate records and participate in regular supervision and team meetings
- A Social Work qualification and registration with SWE England
- Sound knowledge and experience of working within the context of Safeguarding and Child Protection
- Excellent communication and assessment skills, with the ability to identify and manage risk effectively
- A desire to develop practice in Infant Mental Health and passion to drive improved outcomes for infants, children, and their families
