Job Overview:

  • Your primary focus will be on providing expert guidance and support to young people, helping them navigate the complexities of leaving care.
  • By fostering strong relationships and offering tailored advice, you will help individuals overcome challenges and reach their full potential.

Responsibilities:

  1. Conduct thorough Pathway Plan assessments, taking into account each young person's unique circumstances and requirements.
  2. Develop and implement personalized plans, setting achievable goals and milestones to promote progress and success.
  3. Establish and maintain effective communication channels with key stakeholders, including social workers, local authorities, and other support services.
  4. Liaise with colleagues and external agencies to ensure seamless delivery of services, optimizing outcomes for care-leavers.

Requirements:

  • A minimum of 2 years' experience working with care-experienced young people is required.
  • Prior experience in a similar role or a related field is highly desirable.
  • An Enhanced DBS check with barred list clearance is essential.

Estimated Salary:

□ £29,000 - £34,000 per annum, based on qualifications and experience.
