Job Description

We are seeking a dedicated and compassionate Recovery Support Worker to join our Adult Care team at Pennine Care NHS Foundation Trust. As a key member of our clinical team, you will provide high-quality care and support to patients with complex adult care needs.

About the Role

  • You will work closely with patients and their families to identify and achieve individual goals, promoting recovery and independence.
  • Develop and maintain relationships with local community resources and services to ensure seamless care transitions.
  • Contribute to the development and implementation of comprehensive care plans that address the unique needs of each patient.

Requirements

  • You must be a registered social worker with post-graduate level experience in adult care.
  • Demonstrate strong assessment and planning skills, as well as excellent communication and interpersonal abilities.
  • Possess knowledge of relevant legislation, including the Mental Capacity Act.

Benefits

  • A competitive salary range of £32,000 - £40,000 per annum, depending on experience.
  • An attractive benefits package, including 27 days annual leave (rising to 29 days after 5 years) and 8 bank holidays.
