About the Role

We are seeking an experienced and skilled Community Matron to join our team at Berkshire Healthcare Foundation Trust. As a Community Matron, you will play a key role in managing complex patients with multiple chronic diseases, promoting shared decision-making, and encouraging proactive self-management of long-term conditions.

Key Responsibilities
  • Manage a caseload of high-risk patients with complex needs, applying advanced clinical skills and clinical reasoning to ensure good clinical practices and maintain high standards of care.
  • Identify and plan preventative measures and anticipatory care needs, working collaboratively with other stakeholders to improve health outcomes and quality of life for patients.
  • Lead and support community nursing teams, upskilling junior members of staff and promoting a culture of excellence and care.
  • Work collaboratively with other healthcare professionals to develop innovative solutions and improve patient care.
Requirements
  • Registered Nurse (Adult) with a 1st Level degree and IP Qualification.
  • Nurse Independent and Supplementary Prescriber Qualification.
  • Post-graduate Diploma or equivalent in Specialist Practitioner qualification.
  • 5 years post-registration experience in a variety of settings, including hospital, primary, and community care.
  • Operational experience of supporting people with long-term conditions.
