Senior Nurse

We are seeking an experienced Senior Nurse to join our Enhanced Intermediate Care Service team in Bracknell. As a key member of our multidisciplinary team, you will work closely with Physiotherapists, Nurses, Clinical Psychologists, and Community Matrons to provide high-quality care to patients in their own homes.
About the Role:
- Work as part of a strong multidisciplinary team to ensure quality standards are met for patients.
- Support patients to achieve their SMART goals and work with an agreed specific caseload.
- Collaborate with other healthcare professionals to provide comprehensive care and support.
- Contribute to the development of innovative solutions to improve patient outcomes.
