About the Role
We are seeking an experienced and skilled Community Matron to join our team at Berkshire Healthcare NHS Foundation Trust. As a Community Matron, you will play a key role in managing complex caseloads of high-risk patients, applying advanced clinical skills and clinical reasoning to ensure the best possible outcomes.
Key Responsibilities
- Proactively manage a complex caseload of high-risk patients, applying advanced clinical skills and clinical reasoning to ensure the best possible outcomes.
- Work collaboratively with other healthcare professionals to develop and implement care plans that meet the individual needs of patients.
- Provide expert advice and guidance to junior staff members, promoting a culture of excellence and continuous learning.
- Contribute to the development and implementation of policies and procedures that promote high-quality patient care.
Requirements
- Registered Nurse (Adult) qualification and experience of working in a community setting.
- Post-graduate Diploma or equivalent in a relevant field, such as Specialist Practitioner qualification.
- Advanced physical examination and assessment skills, as well as health coaching skills or willingness to develop.
- Excellent organisational and leadership skills, with the ability to prioritise and co-ordinate workload.
- Ability to interpret and analyse complex data from numerous sources, to inform decisions.
