Main Responsibilities

As a Community Health Practitioner, you will work closely with service users, carers, and colleagues to provide person-centred support, helping individuals identify their strengths, personal interests, and goals.

You will be responsible for forming and sustaining trusting relationships with service users, using effective communication skills to engage with them and their families.

With supervision, you will manage your workload efficiently, making decisions within established guidelines and demonstrating a positive, non-judgmental approach to mental health issues.

Requirements
  • Direct personal experience of living with a mental health condition or being a family member/carer to someone who has had a mental health condition.
  • Professional, voluntary, or lived experience of supporting people with mental health needs.
  • Well-developed listening skills and effective verbal communication skills.
