Job Summary

A unique opportunity has arisen to join our well-established Community Nursing teams in Wokingham or Bracknell as a Community Matron/Enhanced Community Nurse Practitioner.

Main Responsibilities

The post holder will work full-time within an integrated community nursing team to proactively manage a complex caseload of high-risk patients, applying advanced clinical skills and clinical reasoning to practice autonomously.

The post holder will promote shared decision-making with individuals regarding their care plan and encourage proactive self-management of long-term conditions to avoid crisis and unplanned hospital admission.

The post holder will work fully integrated into the locality community nursing team, working with other stakeholders to manage complex patients, as well as the wider multi-disciplinary team in various settings to improve health outcomes and quality of life for patients with multiple chronic diseases.

Requirements

The post holder will need to:

  • Have a high level of clinical knowledge and clinical reasoning skills
  • Be able to use advanced assessment skills and expert knowledge to perform comprehensive assessments of physical, mental health, and social care needs
  • Be able to identify and plan preventative measures and anticipatory care needs
  • Have a strong understanding of current National Service Frameworks and the NHS England long-term conditions strategy
Person Specification

The ideal candidate will have:

  • IP Qualification
  • 1st Level degree
  • Nurse Independent and Supplementary Prescriber Qualification
  • Registered Nurse (Adult)
  • Post-graduate Diploma or equivalent - (Specialist Practitioner qualification)

Desirable qualifications include:

  • Leadership/Teaching qualification

The post holder will need to demonstrate:

  • Evidence of relevant continual professional development
  • 5 Years post-registration experience
  • Experience of working in a variety of settings (hospital, primary, and community)
  • Operational experience of supporting people with long-term conditions

Desirable experience includes:

  • Experience of leading change
  • Management experience
  • Experience of working as a change agent to improve patient care

The post holder will need to possess:

  • Case Management skills
  • Advanced physical examination and assessment skills
  • Health coaching skills, or willingness to develop
  • Excellent organisational and leadership skills
  • Ability to prioritise and co-ordinate workload
  • Excellent problem-solving skills to identify and resolve clinical and whole system issues
  • Ability to interpret and analyse complex data from numerous sources, to inform decisions
  • Highly developed interpersonal and communication skills
  • Ability to effectively delegate activities to appropriate staff
  • Ability to effectively negotiate on patient management
  • Ability to engage and develop partnership working with all those involved in the management of long-term conditions
  • Ability to work as a member of a team
  • Ability to work unsupervised and manage own caseload
  • Ability to make clinical and administrative decisions
  • Ability to be goal and outcome-focused when faced with opposition or when working under conditions of pressure
  • Ability to use IM&T support systems
  • Ability to translate best practice evidence and national policy into practice

The post holder will need to be able to travel to and work from multiple sites.
