About the Role

We are seeking a highly skilled and experienced Advanced Practitioner to join our community nursing team. As an essential member of our Urgent Community Response service, you will be visiting patients who are acutely unwell and assessing their clinical and functional needs to reduce unnecessary hospital admissions.

This challenging yet rewarding role requires you to use your advanced practice skills within the four pillars of Advanced Practice to deliver exceptional care to complex patients in the community.

Key Responsibilities
  • Use advanced clinical assessment and diagnostic reasoning to identify patient needs and develop effective management plans.
  • Collaborate with hospital colleagues to develop virtual wards and improve patient outcomes.
  • Work autonomously and make decisions based on evidence-based practice.
  • Communicate effectively with patients, families, and healthcare professionals.
  • Contribute to the development of the trust's policies and procedures.
Requirements

To be successful in this role, you will need:

  • A current professional registration such as NMC or HCPC.
  • A First Level degree or relevant qualifications.
  • MSc Advanced Practice or equivalent Clinical Masters.
  • Recognised teaching/assessing qualification.
  • Evidence of professional/clinical knowledge managing acutely unwell patients in the community care setting.
