Nurse Specialist in Diabetes Care

The Norfolk Community Health and Care NHS Trust Diabetes Team is a dynamic and supportive team, providing expert care for individuals with diabetes in various settings. With a strong focus on patient-centered care, we strive to deliver high-quality services that meet the needs of our patients.
We are seeking an experienced Nurse Specialist in Diabetes Care to join our team, who will work closely with patients, families, and healthcare professionals to provide education, support, and guidance on diabetes management. The successful candidate will have extensive experience in managing individuals with complex diabetes needs and a strong commitment to evidence-based practice.
This is an excellent opportunity for a motivated and caring individual to make a meaningful difference in the lives of people living with diabetes. As part of our team, you will be responsible for:
- Providing expert knowledge and skills in diabetes care
- Managing a caseload of patients with complex diabetes needs, using evidence-based practice and patient-centered goals
- Delivering health education to service users, their families, carers, and other agencies involved in their care management
- Working independently in patients' homes across West Norfolk, with access to the diabetes team remotely
- Providing an expert resource for primary health care teams to provide care and support for people with diabetes in the community setting
We offer a competitive salary of approximately £35,000 per annum, depending on experience, and a range of benefits, including comprehensive training and development opportunities.
