About the Role

We are seeking an experienced Paediatric Diabetes Care Specialist to join our dynamic team at Norfolk Community Health and Care NHS Trust. As a key member of our diabetes service, you will be responsible for providing expert care and support to patients with complex diabetes needs in West Norfolk.

The successful candidate will have extensive experience in managing individuals with diabetes and a strong knowledge of relevant NHS policies, including the National Service Framework (NSF) for Diabetes and National Institute for Health and Care Excellence (NICE).

In this role, you will work closely with primary health care teams to provide care and support for people with Diabetes in the community setting. You will also be responsible for reviewing new referrals to community insulin caseloads and providing advice and guidance to community nursing teams on diabetes management.

Requirements

  • Registration in adult nursing with extensive post-registration experience
  • Recent clinical experience in the management of individuals with diabetes
  • Good communication skills face-to-face and on the telephone
  • Able to travel between bases and hold a full and valid driving license
