About the Role

We are seeking a Paediatric Diabetes Advanced Nurse Practitioner to join our team in West Norfolk. This is a fantastic opportunity to work in a busy nurse-led team, assessing and managing children with complex diabetes and other co-morbidities.

The successful candidate will have extensive post-registration experience in the management of individuals with diabetes and will be able to demonstrate examples of innovative practice. They will also have excellent interpersonal skills and be able to work effectively within teams, demonstrating facilitative leadership skills.

About the Team

We are a growing, friendly, and supportive team who provide care for people living with diabetes in a variety of settings, including clinics, patient's homes and in care settings. We pride ourselves on delivering high standards of care at all times, while embracing change and innovation.

  • You will work within the Diabetes Team providing expert knowledge and skills.
  • You will be responsible for managing a caseload of patients with complex diabetes needs, using evidence-based practice and patient-centered goals to assess, plan, implement and evaluate interventions.
  • The role will include health education to the service users, their families, carers', and other agencies involved in their care management and you will work to promote and encourage self-management wherever possible.
Requirements
  • Registration in adult nursing with extensive post-registration experience.
  • Recent clinical experience in management of individuals with diabetes.
  • Recent clinical experience in management of individuals within Community Care.
Salary Information

We offer a competitive salary of £40,500 - £64,080 per annum, depending on experience, plus benefits including flexible working hours and opportunities for professional development.
