Community Mental Health Nurse/Practitioner

We are seeking a highly skilled and dedicated Community Mental Health Nurse/Practitioner to join our team at Dorset HealthCare University NHS Foundation Trust. As a key member of our Community Mental Health Team for Older People, you will play a vital role in providing high-quality care and support to individuals suffering from mental health illnesses.
Key Responsibilities- Act as a dedicated Care Coordinator for a caseload of patients, providing person-centered, holistic care to aid their wellbeing and recovery.
- Assess, plan, implement, and evaluate programs of care, taking into account the individual's unique needs and circumstances.
- Work collaboratively with a multidisciplinary team, including psychiatrists, psychologists, nursing staff, and support workers, to ensure comprehensive care and support.
- Develop and maintain strong relationships with patients, carers, and other professionals to ensure effective communication and coordination of care.
- Contribute to the development and implementation of care plans, taking into account the individual's goals, needs, and preferences.
- Participate in regular team meetings and contribute to the development of the team's overall strategy and goals.
- Registered Mental Nurse with valid registration with the NMC.
- Experience of working with older adults with mental health illnesses, preferably in a community setting.
- Ability to assess, plan, implement, and evaluate programs of care, taking into account the individual's unique needs and circumstances.
- Strong communication and interpersonal skills, with the ability to work effectively with patients, carers, and other professionals.
- Ability to prioritize and manage a caseload, ensuring timely and effective care and support.
- Competent computer skills, with experience of inputting information on RIO.
