We are seeking a highly skilled and dedicated Community Mental Health Nurse/Practitioner to join our team at Dorset HealthCare University NHS Foundation Trust. As a key member of our Community Mental Health Team for Older People, you will play a vital role in providing person-centered care to individuals suffering from severe mental illness.
Key Responsibilities- Act as a dedicated Care Coordinator for a caseload of patients, providing holistic care to aid their wellbeing and recovery.
- Assess, plan, implement, and evaluate programmes of care, prioritizing and meeting deadlines, and managing complex cases.
- Work collaboratively with carers and other professionals to deliver high-quality care, utilizing effective resource management and risk assessment skills.
- Develop and maintain strong relationships with patients, carers, and colleagues, promoting a positive and supportive team environment.
- Participate in multidisciplinary team meetings, contributing to the development of care plans and strategies to improve patient outcomes.
- Registered Mental Nurse with valid registration with the NMC.
- Experience working with older adults with organic and functional mental illness in an inpatient or community setting.
- Excellent communication and organizational skills, with the ability to prioritize and manage a caseload effectively.
- Ability to work collaboratively as part of a multidisciplinary team, with a positive and proactive approach to delivering high-quality care.
- Competent computer skills, with experience inputting information on RIO.
