Older Adult Community Mental Health Nurse/Practitioner

Dorset HealthCare University NHS Foundation Trust is seeking a highly skilled and dedicated Community Mental Health Practitioner to join our team in Weymouth. As a key member of our Community Mental Health Team for Older People, you will play a vital role in providing high-quality care and support to individuals suffering from mental health illnesses.
About the RoleThis is an exciting opportunity to work in a dynamic and forward-thinking team, providing assessment, treatment, and support to older adults with complex mental health needs. As a Community Mental Health Practitioner, you will work closely with a multidisciplinary team, including psychiatrists, psychologists, nursing staff, and support workers, to deliver person-centered care and promote recovery.
Key Responsibilities- Assess, plan, implement, and evaluate programs of care for older adults with mental health illnesses.
- Conduct risk assessments and develop management plans to ensure the safety and well-being of service users.
- Work collaboratively with the multidisciplinary team to provide comprehensive care and support.
- Develop and maintain effective relationships with service users, carers, and other professionals to ensure seamless care coordination.
- Participate in the development and implementation of care plans, taking into account the individual needs and preferences of service users.
To be successful in this role, you will need:
- Registration with the Nursing and Midwifery Council (NMC) as a Registered Mental Nurse (RMN).
- Experience working with older adults with mental health illnesses, preferably in a community setting.
- Excellent communication and interpersonal skills, with the ability to work effectively with diverse groups.
- Strong assessment and planning skills, with the ability to prioritize and manage complex cases.
- Ability to work autonomously and as part of a team, with a flexible approach to work.
